PRN Pharmacist

Pocahontas Memorial Hospital is seeking a Per Diem Pharmacist for Monday-Friday, day shift coverage and occasional on-call for after hours and weekends. Responsibilities include: verifying physician orders, overseeing work completed by Pharmacy Tech, ensuring proper handling of medications, maintaining standards set forth by PMH, and demonstrating competence to provide pharmaceutical care to different patient populations. Candidate must possess strong interpersonal communication and computer skills as well as be self-motivated. Requirements include Bachelor of Science in Pharmacy or PharmD and license to practice Pharmacy in West Virginia.

Staff Pharmacist
Location: Ohio Valley Medical Center Inpatient Pharmacy Department
                2000 Eoff Street
                Wheeling, WV 26003
•2 years of hospital experience preferred, but not required
•Must have active WV pharmacy license
•Must be willing to make clinical interventions or interact with physicians
•Interprets medication orders and maintains accurate medication profiles with dose adjustments as necessary for different age groups and based on clinical indicators approved by P&T Committee.
•Checks Pyxis restocking bins prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed. 
•When necessary, prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.
•Answers questions and provides information to physicians, nurses, patients, and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc.
•Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste.

Clinical/Oncology Pharmacist

Location: Ohio Valley Medical Center Inpatient Pharmacy Department
                 2000 Eoff Street
                 Wheeling, WV 26003
•BS or PharmD in pharmacy
•Valid pharmacy license in WV
•3 years hospital experience preferred
•Excellent communication skills
•May be required to work different shifts
•Clinical programs will ensure the safe and effective use of medications through assessment of optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services. The Clinical Pharmacist will implement systems to monitor drug therapies when appropriate, provide dosing and IV to Po conversion recommendations, discharge counseling on selected patients, and work with the related medical staff to find alternative when needs to ensure the best patient care is being given. Assisting the Director of Pharmacy with projects and scheduled staffing coverage will also be required. Preparation and oversight of oncology regimens requires focus on detail and knowledge of chemotherapy techniques
•A person with oncology experience is preferred to fill in the workflow for a growing oncology outpatient service. Duties will include entering chemotherapy regimens into Meditech and GeniusDoc, calculating doses, mixing and checking mixed IV preparations as well as any pre-medication treatments.
•This will be a split position, but may evolve into a full-time oncology position.
